A minibus taxi and two cars collided on the N4 near the Solomon Mahlangu offramp, Netcare 911 spokeswoman Santi Steinmann said.
"The minibus overturned and came to a standstill on its side. One of the vehicles came to a standstill on top of the other vehicle," she said.
Rescue services were working to free a critically injured patient who was trapped in the wreckage. The other seriously injured patient was taken to hospital.
Traffic was severely affected.
